Immunization Records

North Carolina law requires all children in the state to receive certain immunizations.  To enroll in any public school, all  students must provide a copy of their immunization records to be included in their permanent records file.  Immunizations should be up-to-date and verified by an authorized health care provider.  The immunizations are listed below and more information can be located by clicking here:

    • Diphtheria, tetanus and pertussis
    • Polio
    • Measles
    • Mumps
    • Rubella
    • Haemophilus Influenzae type B (Hib)
    • Hepatitis B (Hep B)
    • Varicella (chickenpox)

Grade 6  – Tdap Booster is required!

A booster dose of Tdap vaccine is required for all public school students entering  Grade 6, if five years or more have passed since their last dose of tetanus/diphtheria toxoid.
